TGA

When the great vessels are transposed, the aorta becomes anterior to the pulmonary artery. The new alignment of the great vessels occupies a narrower space when viewed in an AP CXR, this will render the mediastinum narrow. In addition, TGA may be associated with absent Thymus, also contributing to narrow mediastinum.

The right and left ventricles will dilate due to increase volume load causing cardiomegaly.
