TAPVR

In this anomaly, the pulmonary veins meet in a confluence behind the left atrium. The confluence then drain through a venous structure into a systemic venous vessel or chamber. The most common is drainage to the SVC through a vertical vein (left SVC), which connects to the innominant vein to the SVC. This results in a shadow above that of the cardiac shadow on AP CXR resulting in a figure of 8 or snow man configuration.

The pulmonary venous confluence could also drain into the right atrium directly, to the coronary sinus or to the IVC.
